With this survey we would really like to find out how you feel about the role of the church and its relationship with the surrounding community as it relates to drug addiction.
The results of this survey will allow us to recognize the positive link between faith-based groups such as the church and the surrounding community with the hope to build on the strengths of both. With a focus on drug addiction, wellness, and health promotion our hope is to encourage beneficial partnerships, strengthen relationships and aid the community in healing the body, mind, and spirit.

  • Women over the age of 40should get yearly mammogram screening
